Acceptable Use Policy SMCPS
All students are required to have an acceptable use policy agreement (AUP) on file. This is to be completed by the PARENT or GUARDIAN of the student.

Bus Stop Change Request Form
The Department of Transportation receives numerous requests for new bus stops and changes to existing bus routes throughout the school year.

Meal Application Form
All families must fill out a new meal benefit form as soon as possible. Filling out the form early ensures your benefits stay current.
Student Meal Account Refund or Transfer
This form can be used to request a student meal refund from the Department of Food and Nutrition Services.

Volunteer Registration
Both new and previously registered volunteers must complete a new volunteer application every school year.
GoFan Online Ticketing
SMCPS primarily operates using an online ticketing process through GoFan for ticket sales. Online tickets can be purchased through GoFan at www.gofan.co or via the GoFan App.
